To expedite your visit to Dr. Reuter’s office you can print and fill out the following forms prior to being seen or treated. Please read each form very carefully, fill in as much information as you can, and if you have any questions about the information requested in these documents, don't hesitate to contact Dr. Reuter.

Treatment of Minors:

Dr. Reuter is happy to treat children and teenagers for short term relief of symptoms. For those patients under the age of 18, a parent or guardian must be present for the full duration of the appointment, and will be required to sign a consent form in our office prior to treatment.
